Richland County restaurant inspections: Critical violations June 2-6

The following restaurant inspections with critical violations were conducted by Richland Public Health June 2 - 6:

• Tacos & Margaritas, 288 Lexington Ave., Mansfield, June 2. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Soda dispenser nozzle had visible residue. TCS foods not being cold held at the proper temperature (critical, corrected). Walk-in freezer had ambient temperature of 43 degrees. TCS items at 42-43 degrees moved to other coolers or put in ice pans. Person in charge (PIC) voluntarily discarded three pounds of beans, five pounds of marinated steak, and two pounds of cooked ground beef.

• Subway #30929, 1521 Lexington Ave., Mansfield, June 2. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand sink on front make line used to rinse off utensil. Hand sinks are to be used for hand washing only. Equipment food-contact surfaces or utensils are unclean (critical, corrected).

Ice machine ice production area has visible black residue. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Probe for thermometer was not sanitized prior to attempting to take temperature. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Ware washing sink wash had temperature of 102 degrees. 

• Mansfield Correctional Institution/Camp, 1150 N. Main St., Mansfield, June 2. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Thermometer was taken out of case and inserted into butter without first sanitizing thermometer. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Sanitizer was measured at only 100 ppm. 

• Mansfield Correctional Institution - Main, 1150 N. Main St., Mansfield, June 2. Improper washing of hands and arms (critical, corrected during inspection). Two employees observed washing hands and contaminating clean hands on dirty sink handle. Handwashing sink is being used for purposes other than handwashing (critical, corrected). Food worker observed filling wash basin from hand wash sink. TCS foods not being cold held at the proper temperature (critical, corrected).

Six pans of cold-holding macaroni salad (cooked pasta with dressing) just put on serving line were measured at 60 degrees. Listed items were taken of serving line and refrigerated in cold holding unit. Food contact surfaces not easily cleanable (critical repeat, corrected). Large number of pots and pans observed dented .Facility is scheduled to have new pots arrive for use.

• Besta-Fasta Pizza, 289 Marion Ave., Mansfield, June 2. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed an opened container of turkey lunch meat in the cooler without a date mark.

• Doc's Deli, 424 Glessner Ave., Mansfield, June 2. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed a tub of cookie dough at 55 degrees. PIC voluntarily discarded approximately half a tub of cookie dough. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed opened packages of American and pepper jack cheese and capocollo in the refrigerator without a date mark.

• Domino's Pizza, 625 Lexington Ave., Mansfield, June 3. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical repeat, corrected during inspection). PIC unable to locate where employee was informed in a verifiable manner of their duty to report. Food contact surfaces not easily cleanable (critical, corrected). Scissors on make line with food residue observed are not able to be separated to allow cleaning between blades. PIC removed from service.

• The Butcher Shoppe, 279 S. Diamond St., Mansfield, June 3. No safe handling instructions on packaged raw meat (critical repeat). Observed raw patties with no safe handling instruction. Correct by June 10. Food not properly protected from contamination by separation, packaging, and segregation (critical). Observed meat on shelves in walk-in cooler to be uncovered, Correct by June 10.

• Kroger Company #N518, 1240 Park Avenue West, Mansfield, June 3. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected during inspection). Observed ready-to-serve turkey bacon on the bottom shelf in the cooler below raw turkey bacon. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected). 

Observed packages of partially-cooked smoked turkey on the shelves above ready-to-eat hams in the cooler. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Observed the sanitizer inside the three compartment sink in the deli at 100 ppm. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Observed the wash solution inside the three compartment sink in the deli at 83 degrees.

• What's The Skoop, 408 S. Diamond St., Mansfield, June 3. No person in charge present in food facility during inspection (critical). Thirty days to have required training in personnel in place.

• Friends Convenient Mart, 1605 Fourth St., Ontario, June 6. Presence of live rodents (critical repeat). Observed mouse droppings in the back storage area. At time of re-inspection, PIC states pest control has set traps both inside and outside. Attempt was made to clean mouse dropping, however, continued evidence of current activity observed. 

• Wayne's Country Market, 2440 Lexington Ave., Mansfield, June 6.  PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Two locations in facility had sanitizer dispensed at less than manufacturers recommended strengths. Food not properly protected from contamination by separation, packaging, and segregation (critical repeat, corrected). Raw bacon observed stored above fully cooked sausage in retail reach-in cooler.

Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical repeat, corrected). Three-compartment bakery sink had sanitizer strength of 0 PPM. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected). Sanitizer bucket made from produce three-compartment sink had 0 PPM. Manual wash solution not being maintained at 110 degrees or above (critical, corrected). Wash solution in bakery three-compartment sink in use had temperature of 104 degrees. No air gap or approved backflow prevention device on the plumbing system (critical, corrected). Hose reel connected to two compartment sink was not equipped with back flow.

• Cinemark 14, 2355 Walker Lake Road, Ontario, June 6. Person in charge did not ensure employees are properly cooking TCS foods and/or using thermometers as required (critical). PIC states frozen mozzarella sticks, pizza and pre-cooked chicken cooked in oven are not checked for proper internal cooking temp of 135 degrees prior to serving.

• Donatos Pizza, 855 Lexington Ave., Mansfield, June 6. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected during inspection). Pizza dough observed in walk-in freezer uncovered and exposed to splash, dust or sneeze.
