Topgolf Richmond, 2308 Westwood Avenue – The following violations were reported during a routine inspection: temperatures of portioned turkey, portioned steak and shredded lettuce are elevated because the containers are stacked high in the prep unit; grill cleaner incorrectly stored near pretzel salt and water; observed grime buildup around drains at drink/caddy station on second and third floors.

Toast, 7007 Three Chopt Road – The following violations were reported during a follow-up inspection: interior of ice machine has mildew; temperatures of banana pudding and dressing in glass door reach-in are elevated; drip trays and fryer cabinet doors are soiled; walk-in door is dirty (interior and exterior); spilled kitty litter found on outside of storeroom floor; fan grills and ceiling inside walk-in are dusty.

Roots Natural Kitchen—Richmond, 6602 West Broad Street – The following violations were reported during a routine inspection: hot water at hand sink on front line doesn’t reach the required temperature; observed food debris greater than one day old/stored clean on knives in knife holder on wall in prep area and on vegetable slicers on shelf under prep table and on shelf near dish area; temperatures of cut tomatoes, tofu, cooked broccoli and sweet potatoes are elevated because they are all improperly cooling in very deep containers with saran wrap; temperature of purple rice on steam table is too low; temperatures of kale, arcadian mix and spinach in salad display case are elevated; to-go boxes on front line are not being stored inverted; observed debris on shelves, outside of equipment and on shelf in prep area; observed debris/buildup on floor corners where convection oven is located, on floor under equipment (not in use) across from to-go reach-in unit, and on floor under prep sink in back kitchen; observed buildup/stains on wall behind convection oven in back kitchen.

Krispy Kreme Doughnuts, 4906 West Broad Street – The following violations were reported during a routine inspection: dishes found in multiple hand sinks (hand sinks are for handwashing only); hand sink in dough-making area doesn’t have hand soap or paper towels; observed small flies in the women’s restroom; drink stirrers incorrectly stored in water; three-compartment sink is in disrepair; floor tiles throughout kitchen, especially by mixers, are cracked.

Starbucks Coffee Company, 1601 Willow Lawn Drive – The following violations were reported during a routine inspection: single-service cups are overfilled in dispenser; exterior of freezer is soiled; cabinet under front prep sink is dirty; dish machine is leaking onto floor; appears that sanitizer was hooked up to the dish machine instead of soap.

The Beach House, 4040-B Cox Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit/diarrhea; no handwash signage found in either men’s or women’s restrooms; sliced deli chicken incorrectly stored next to and on top of a pan of raw chicken; a “clean” cutting board has sauce stuck on it; interior of ice machine has patches of mold; can opener blade is dirty; temperature of sliced tomatoes on the line are elevated because they are overfilled; multiple boxes of food incorrectly stored on freezer floor; cutting boards at make tables are scratched; one light is burned out under the hood; fan grates in freezer and beer cooler are dusty; floor and wall junction in the dish area is dirty; bar floors are soiled in areas; old lemon wedges and trash found behind equipment; kitchen ceiling is yellowed and ceiling vents appear rusty; cook line floor is pitted; beer cooler floor has a large dent at the entrance.

Sushi Hibachi Bistro, 10490 Ridgefield Parkway – The following violations were reported during a routine inspection: observed an employee eating on the cook line; observed an employee rinse hands after eating at three-compartment sink (wash hands with soap at hand sink); dried food debris found on two peelers hanging up over prep table; escolar incorrectly thawing in reduced-oxygen packaging in reach-in.

Tropical Smoothie Café, 11321 Nuckols Road – The following violations were reported during a routine inspection: observed an employee using improper handwashing procedures; hand sink near three-compartment sink doesn’t have paper towels; observed grime and grease on blade, holder and surfaces of can opener; lower shelves of prep tables are rusty; single and small prep unit cutting boards are heavily scratched and stained; observed dust and dried food debris on wire storage racks, oven, inside and outside of prep units, towel dispensers, counters, blenders and inside utensil bins; back door is rusted and in disrepair; grease receptacle is heavily rusted and the lid is removed; observed dirt and dried food debris on floors, walls and plumbing pipes.

KFC/Taco Bell, 8340 Brook Road – The following violations were reported during a routine inspection: facility doesn’t have a written policy for how employees are to clean up bodily fluids; food debris found on scoops and other utensils hanging on wall; several pieces of equipment are in disrepair (lowboys under hot holding line, reach-in by cook line, hot holding unit by drive thru, service line across from fryers, and sink by drive thru); service lines have ice buildup with pans stuck in the ice; observed grease or grime buildup on hot holding units, racks in walk-in cooler, handles of units, and pipes and sides of equipment on cook line; there is low grouting and cracked floor tiles throughout the facility causing standing water and food buildup; drain at hand sink by walk-in cooler has heavy buildup of food and grime; observed grease buildup behind cook line; ceiling and wires above prep area are dusty.

Starbucks Coffee Company, 5001 Huguenot Road – The following violations were reported during a routine inspection: conspicuous thermometers not found in all units; single-service cups are overfilled in dispenser; dish machine doesn’t have heat tapes or max registering thermometer; wall by mop sink is in poor condition.

Arby’s at Tuckernuck Square, 9080 West Broad Street – The following violations were reported during a routine inspection: facility couldn’t provide a certified food protection manager certificate during inspection; paper towel dispenser at hand sink on front line isn’t working; observed food debris greater than one day old on multiple scoops and one end of scoop was sticky even though it was stored clean; wagyu burgers at fry station are not being cooked to the proper temperature; temperature of burgers cooked last night in lowboy unit are elevated; temperature of fries on warmers at fry station are not hot enough; temperatures of cheese, corned beef, turkey, pre-portioned deli meats, and lettuce in sandwich unit are elevated; doors for sandwich unit are in disrepair; observed leak coming from right bottom corner of sandwich unit; incorrect discard date marked on pot pie in lowboy unit and walk-in cooler; temperatures of au jus, sliced tomatoes and rotisserie chicken are elevated because proper cooling methods were not followed; observed multiple small flies throughout the kitchen and front line, especially under hot box where there is a piece of bread on the floor; to-go trays are not stored inverted; milk shake machine is broken; multiple cutting boards in prep area are heavily scratched; observed debris on shelves and in cabinets under soda machine, under prep table, on shelf in walk-in cooler, on shelves on front line, and on shelves and in drawer in prep area; fan guard in walk-in cooler and hood filters above convection oven are dusty; observed heavy grease under fryers on fry station, in between equipment on cook line, and on wall next to fryers; faucet at three-compartment sink is leaking; observed debris/trash on floor corners throughout the facility, especially under equipment; there is low grouting throughout facility; floor tiles in both walk-in coolers and in walk-in freezer are cracked.

Regal Cinemas—Short Pump, 11650 West Broad Street – The following violations were reported during a routine inspection: did not observe employees working concessions wash their hands at all during inspection; both concession hand sinks are out of soap and one lacked paper towels; dishwasher is dispensing 0 ppm chlorine when tested; gasket in under-counter freezer at concessions has tears in it.
